Terms of Use for StellarQ My, Patient Report Service

  • The use of the StellarQ My service is only permitted if the well-being services county responsible for the care authorizes the use of the service.
  • The use of the StellarQ My service requires acceptance of the terms of use.
  • A patient using the StellarQ My service has been informed about the storage, disposal, and collection of health information contained in the service.
  • The unit responsible for the patient's care has the right to transfer information self-recorded by the patient to the patient information system.
  • Authentication to the service is done using strong authentication.
  • The storage of information follows Finnish law and appropriate data security regulations.
  • The well-being services county responsible for the care owns the generated information.
  • The well-being services county responsible for the care can terminate the use of the StellarQ disease management, which will also result in the termination of the use of StellarQ My service.
  • The storage and disposal of information follow Finnish law.
  • Anonymous research data and statistics can be generated from the information of all patients using the StellarQ My service and provided to third parties.

Consent for Service Usage

I have familiarized myself with the privacy policy and terms of use for StellarQ My service. I understand that giving consent is voluntary and I have the right to refuse. I can also stop using the service at any time.

I consent to the following:

  • The well-being services county responsible for the care can view and modify the information I have saved in the StellarQ My service.
  • The well-being services county responsible for the care can copy the information I have saved in the StellarQ My service, as applicable, to other information systems of the well-being services county.

Termination of Service Usage

  • I have the right to stop using the service at any time. The well-being services county responsible for the care is responsible for the storage and deletion of already saved information.
